If you are seeking access to your personal documents, please include a certified copy of your passport or driver’s licence or other form of photographic identification.

APPLICATION FEE

The application fee to be paid is AUD$29.60.

For payments by cheque: A cheque addressed to the University of Divinity should be posted to the address provided above, along with a hard-copy of the completed form. Ensure that the cheque can be clearly identified as payment for your Freedom of Information request.
